Citrus County, Florida (“County”) invites interested parties to submit a Proposal to provide roadway resurfacing Work for various roadways throughout Citrus County. The work includes, but is not limited to, furnishing all equipment, labor, and material for resurfacing the proposed roads using the methods listed in this Request for Proposal. MISCELLANEOUS REQUIREMENTS • Milling-County to keep all millings. Deliver millings to the Road Maintenance Facility located at 5272 W. Homosassa Trail, Lecanto, Florida. Hauling of the millings will be incidental to the contract. • Milled “local” roads can be left open to traffic for a maximum of 24 hours provided there is proper MOT notifying the public of the uneven pavement. Milled “Minor” or “Major” collector roads can NOT be left open to traffic overnight. • The asphalt to be provided for the project shall be FDOT pre-approved and identified on the current FDOT Asphalt Mix Design Plant Assignment Sheet. • Bidder must presently have at time of award of a contract; personnel, and equipment necessary to perform the Work outlined in this Invitation to Bid. • Bidder must presently have or will procure at time of award of a contract, any and all materials necessary to perform the Work outlined in this Invitation to Bid. • Bidders and/or Subcontractors must be pre-qualified by the Florida Department of Transportation and submit a copy of the FDOT Pre-Qualification Documentation at the time of Bid Submittal for: • Grading • Grassing, Seeding, and Sodding • Hot Plant-Mixed Bituminous Courses • Pavement Marking • Bidder must have an individual onsite during all phases of work associated with this project including but not limited to roadway preparation, asphalt milling, paving, striping, and sod placement who is certified in work zone safety (Maintenance of Traffic (MOT) Intermediate Level) in accordance with the Florida Department of Transportation requirements. Provide MOT certification. • AIA document G702 (Required Form): The AIA G702 form serves as both the Contractors application and the architects certification. Its use can expedite payment and reduce the possibility of error. This document is often referred to as an AIA billing form, progress billing form, payment app, or pay app. The County’s requires this form to be used when the Contractor is submitting their application for payment. A sample form is attached at the end of this Solicitation Document. The form requires Contractor to certify and will need to be notarized before submitting to County. Bonds required.
